Anyone under the age of 18 years old who wants to get pierced will need to bring in this form completed and notarized. There are no exceptions.The form must be notarized by a parent/legal guardian, and both the minor and the parent/legal guardian must show ID's at time of service.

If you received a piercing from anywhere in the State of Florida and it becomes infected, this form must be sent to the health department. We often get clients who are pierced elsewhere and come to us for help. We’ll gladly help and submit the form for you.
